When you are referred to a dialysis facility, you will be asked to provide insurance information and a copy of your card so that your health plan will be billed correctly. Review all the materials provided by your health plan and/or Medicare and make notes of important points for yourself. Talk to your social worker or financial coordinator about any questions you have. He or she may suggest you talk to your health plan’s customer service staff, which can answer specific questions about your policy.
